Meeting Type & Student Info
Student Name / Birthdate — Auto-populated from SEIS/SIS. Verify before sending — name must match exactly across all IEP documents.
Meeting Type checkboxes — Check ALL that apply. A single notice can cover multiple purposes:
• Initial — First IEP after eligibility determination
• Plan Review — Annual IEP update
• Reevaluation — Triennial or parent-requested evaluation review
• Transition Planning — Required at age 16; CA best practice recommends age 14
• Pre-Expulsion — Required before any disciplinary change of placement
• Interim — For students transferring from another district
Address — Parent's current mailing address from SIS. ⚠ Verify it's current — all IEP notices must reach the family. A returned notice = procedural violation.
Date / Time / School / Room — Must be scheduled at a mutually agreeable time. If the parent cannot attend at the proposed time, document all reasonable efforts to reschedule.
Anticipated Team Members
List all expected participants by title (names of participants is not recommended). Required team members for most IEP meetings:
• Administrator/Designee (LEA Representative) — Must have authority to commit district resources
• Special Education Teacher — Who knows the student's curriculum and disability
• General Education Teacher — If the student is or may be in general education
• Parent/Guardian — Must be given meaningful opportunity to participate
Student — Required for transition IEPs; encouraged for all IEPs. Secondary students age 15+ should attend.
Psychologist / Specialist / Other — Include any related service providers (SLP, OT, PT, counselor) whose service area will be discussed.
⚠ A required member can only be excused with written parent consent AND if their area will not be significantly discussed. Get this in writing before the meeting.
Parent Response Section
The parent must return this section. Every checkbox option protects the district's procedural record:
YES — Attending in person — Standard attendance confirmation.
YES — Teleconference — District must accommodate when feasible. Document the technology used.
Interpreter needed — ⚠ Must be provided at NO cost when checked. Never use the student or a sibling as interpreter.
Cannot attend — reschedule — District must make a good-faith effort. Document all scheduling attempts.
Consent for outside agency — Required before inviting Regional Center, DOR, or other agencies to a transition IEP (FERPA/IDEA protection).
NO — Permission to hold without me (CFR 300.322d) — Must be in writing. IEP documents must still be sent to the parent for signature after the meeting.
NO — Sending a representative — That person must have authority to make educational decisions. IEP still goes to parent for signature.
Signature / Date — Keep the returned form in the student file as documentation of parent notification.
